You will need to provide the following information.

- Description of your loss
- time and date of incident, location, and a detailed description of your damages
- Vehicle information (for traffic accidents only)
- current location of the vehicle, owner information, driver information, type of car (year, make, model), and license plate number
- Report information
- author, title (if any), and date
- Additional comments
- including anything else you would like your insurer to know, such as injuries, witnesses, etc.
